SPARK COVID WAIVER

As a participant or as a parent/guardian of a participant of SPARK Basketball & Cheerleading at Jackson Park Baptist Church, I recognize and acknowledge there are certain risks of physical injury and I agree to assume full risk of any injuries, including damages or loss which may be sustained as a result of participation in activities associated with SPARK Basketball & Cheerleading. I agree to waive and relinquish all claims against Jackson Park Baptist Church and its faculty members from any and all claims resulting from participation in the programs offered. I acknowledge the contagious nature of COVID-19 and any other contagious diseases and viruses and voluntarily assume the risk that I and/or my children may be exposed to or infected by COVID-19 by attending and participating and that such exposure or infection may result in personal injury, illness, disability, and/or death. I understand that the risk of becoming exposed or infected by COVID-19 and other contagious diseases and viruses, as always, may result from the actions, omissions, or negligence of myself and/or others, including, but not limited to, employees, volunteers, and program participants and their families.

Child MUST attend one of the evaluation days in order to be placed on the appropriate team. Please choose which day the child will attend. Evaluations take approximately 15 minutes. Don't forget to mark you calendar!

SPARK 24/25 Basketball and Cheerleading Season
Kindergarten (must be 5 years old by August 31, 2024) through 6th Grade
Cost for the 24/25 SPARK Season is $40 per child. Uniform Jersey and shorts are included in this cost

In order to be placed on a team, and to be fitted for a uniform, your child MUST attend one of the evaluation dates. This is very important! We can not guarantee your child a spot on a team if they have not completed this step. Evaluations take approximately 15 minutes.

Registration / Evaluation dates are:
Saturday, October 12 9am-12pm
Tuesday, October 15 6pm-8pm
Thursday, October 17 6pm - 8pm
Saturday, October 19 9am-12pm

Practices begin the week of December 2, 2024. Practices will be held on Monday, Tuesday, Thursday or Friday to be determined after all the evaluations have been completed and teams set up. Your child will have practice on ONE of those days for ONE HOUR. You will be notified of your child's practice day and time in November.
